Submitting a contact or referral form does not guarantee an appointment. Our team will follow up to confirm a time that works. We reserve the right to decline service if our scope of practice doesn't match your needs — and we'll do our best to refer you to someone who can help.

We ask for at least 24 hours' notice for cancellations or rescheduling. Late cancellations or no-shows may be subject to a fee.

We direct bill most major Canadian extended health insurers, WSIB, and motor vehicle accident claims. Coverage is determined by your individual plan. Any portion not covered by your insurance is your responsibility at the time of service.

Insurance reimbursement is between you and your insurer. While we make every effort to verify coverage and submit claims accurately, we cannot guarantee reimbursement.

By providing your contact information, you consent to receive appointment confirmations, reminders, and follow-up messages by phone, email, or text. You can opt out of non-essential communication at any time by contacting us.
